Person-Centered Care: Ensuring Your Loved One is Seen and Known

July 29, 2025 @ 6:00 pm - 7:00 pm

When someone you love is living with dementia, what matters most is that they are treated as a whole person—not just a diagnosis. Join us for a practical, uplifting session focused on person-centered care: what it is, why it matters, and how you can help create it.

Together, we’ll explore:

✅ What person-centered care really looks like

✅ When and how to advocate for it at home or in a care setting

✅ How to create a Personalized Care Profile that highlights your loved one’s preferences, routines, and life story

✅ How to build a simple Memory Book to support connection and reduce anxiety

✅ How to partner with long-term care staff or home care aides to bring these tools to life

✅ What to do when the relationship with staff is difficult—and how to navigate that with compassion and clarity

Whether your loved one is at home or in memory care, this workshop is designed to empower and encourage you with real-life tools, ideas, and support.

You know your loved one best. Let’s make sure everyone else does too. 💛
